New Patients

Our practice is committed to providing you and your family with safe, gentle, and high-quality dental care. We understand that you or your child may feel anxious about visiting the dentist. We are sensitive to your needs, and it is our goal to make you feel comfortable visiting our clinic while providing the best care possible.

Sliding Fee Discount Program

Community Dental Care is committed to providing access to dental care for all patients, regardless of their ability to pay. A discounted/sliding fee schedule is available for eligible patients and is based solely on family size and household income.

No patient will be denied access to services because of an inability to pay.

Patients may apply for the Sliding Fee Discount Program at any time. Staff are available to help patients complete an application and answer questions about eligibility. To request an application or learn more about the program, please contact any Community Dental Care location.

Before your first appointment, you can fill out the Patient Forms ahead of time. If you are not able to print and fill out the forms before your appointment, we ask that you please arrive 15 minutes before your scheduled time to fill out new patient paperwork.

  • List of medications

  • Your completed patient information form  

  • You will need to bring payment for any co-pays or balances owed

YOUR FIRST VISIT IS ALL ABOUT YOU: YOUR COMFORT, YOUR HAPPINESS, AND YOUR HEALTH

During your first visit, we will review your dental and medical history forms, and take X-rays of your teeth and mouth. A hygienist will gather information and will likely clean your teeth. One of our dentists will meet with you to discuss your X-rays, help you create your individual treatment plan, and answer any questions you may have.

REGULAR CHECKUPS ARE AN IMPORTANT PART OF MAINTAINING A HEALTHY SMILE!
