Group plan through HealthPartners.
Community Dental Care pays 80% of the base plan for the employee, and 50% of dependents coverage on the plan. Two buy-up options are offered.
Community Dental Care will contribute up to $300.00 per year to any employee that has a qualified high deductible health plan. This contribution is pro-rated and breaks down to $12.50 per pay period.
Employee, spouse, domestic partner, and children (under age 26 living at home) are provided with free routine preventive and basic care through the clinic. Major work is discounted of 50% for work in this clinic.
Up to $500 credit per family on major work including fixed and removable proths, ortho, TMJ, which can be used at a specialist’s office.
The benefit amount is an employee’s estimated annual salary (based on previous year), and it will be paid to the employee's beneficiary. Community Dental Care covers the entire premium for this policy.
Long Term Disability
If an employee cannot work in his/her job because of injury or illness lasting more than 3 months, the employee will be paid 60% of his/her wage. Community Dental Care covers the entire premium for this policy.
Up to $130 per year is allocated for uniform shoes and scrubs.
401(k) Retirement Investment and ROTH 401(k)
Employee may defer compensation, pre-tax for regular 401(k), post-tax for ROTH 401(k), up to the annual federal limit for 401(k) investment plans. Community Dental Care will match employee contribution at 50% of the first 2% of income.
Employee is eligible to enter the plan during the first quarter after 6 months of employment.
Community Dental Care recognizes the following paid holidays the first month after 60 days of employment:

Accrual starts from their date of hire. A maximum of 160 hours may be accrued to be used for time off.
These additional benefits are available to employees. Employee pays 100% of the premium out of payroll. All policies are offered under a reduced group rate

The following benefits are offered to dentists only:
Dentists have an annual stipend in the amount of $2400 to cover professional fees.
Coverage is immediate upon hire.
This is a group policy with CNA for liability coverage of $2,000,000 per incident and $3,000,000 in aggregate claims.
